Request Your Medical Record
To request a copy of your medical record, print out and complete the Authorization for Release of Protected Health Information. Mail the completed form to:

X-Rays & Other Images
You may request a copy of your CT scan, X-ray, or other images by contacting the Diagnostic Imaging Department at 802-748-7567.
If you also need a copy of the radiologist’s report, please inform us when you call.
