Community Health Fund Grant App Now Open

ST. JOHNSBURY, VT (September 12, 2024) – Northeastern Vermont Regional Hospital (NVRH) is excited to announces that the application for the NVRH Community Health Fund is now open. The NVRH Community Health Fund ensures continuous funding for community health programs and services that help improve the health and wellbeing for all people in the community.

The NVRH Community Health Fund Advisory Committee, which includes representation from NVRH leadership, NVRH trustees, and the greater community, will review applications and select the grantees. Last year’s recipients included H.O.P.E., The St. Johnsbury Community HUB, Lyndon Institute, NEK Community Growth Center, Inc. and Northeast Kingdom Youth Services.

“The recipients of the Community Health Fund mini grants are able to engage in real impact,” NVRH VP Marketing and Community Health Improvement Diana Gibbs said. “For example, as part of the NEK Community Growth Center’s initiative to offer education and health screenings for families with children in the area, ABC & LOL Child Care Center and Preschool welcomed dentist and NVRH Corporator Courtney Kozlowski in to teach children the importance of dental care. And these kids learned preventative skills that will make a difference to their health – long term.”

The timeline for a Community Health Fund mini grant application process is as follows:

  • 12 – Oct. 18, 2024: Grant opportunity announcement and proposal development period
  • 19 – Nov. 15, 2024: Community Health Advisory Committee Grant Review Period
  • Dec. 1, 2024: Grantee award notification deadline
  • Dec. 1, 2024 – Sept. 30, 2025: Implementation of project
  • Oct. 31, 2025: After Action Report due

The application process includes a one to three page narrative, a budget form, letters of support for the project from at least two community partners and a copy of tax exempt status from IRS (for non-profits). Please note that applicants may be asked to supply additional information not outlined in the grant application.

Now in its thirteenth year, the Community Health Fund, which is sustained by generous donors in conjunction with NVRH’s Community Benefit funding, has awarded over $115,000 total in mini grants.
