Patient Center Medical Homes: A new way to practice medicine. The Vermont Blueprint for Health is a new way of looking at the practice of medicine and chronic care. NVRH Corner Medical, Kingdom Internal Medicine and NVRH St. Johnsbury Pediatrics and the Northern Counties Health Care Health Centers of Danville Health Center, Concord Health Center, and St. Johnsbury Community Health Center are using innovations, tools, clinical guidelines and best practices to deliver effective, proactive care – and to involve patients in managing their own chronic conditions. All these medical offices are recognized as Patient Centered Medical Homes by the National Committee for Quality Assurance (NCQA).
Vermonters learning to live healthier. The Blueprint works to help Vermonters who have chronic conditions stay as healthy as possible – improving their quality and enjoyment of life and avoiding the need for complex care later when illness is harder to treat. The Healthier Living Workshops are sponsored by the Blueprint. These self-help workshops teach individuals to manage their chronic conditions and improve their health.
