Blueprint/Medical Home

The Vermont Blueprint for Health is a vision, a plan and a statewide partnership to improve health and the health care system for Vermonters. The Blueprint provides the information, tools and support that Vermonters with chronic conditions need to manage their own health – and that healthcare providers need to keep their patients healthy. The Blueprint is working to change health care to a system focused on preventing illness and complications, rather than reacting to health emergencies.

A new way to practice medicine. The Blueprint is a new way of looking at the practice of medicine and chronic care. NVRH Corner Medical, Kingdom Internal Medicine and NVRH St. Johnsbury Pediatrics and the Northern Counties Health Care Health Centers of Danville Health Center, Concord Health Center, and St. Johnsbury Community Health Center are using innovations, tools, clinical guidelines and best practices to deliver effective, proactive care – and to involve patients in managing their own chronic conditions.  All these medical offices are recognized as Patient Centered Medical Homes by the National Committee for Quality Assurance (NCQA).

Vermonters learning to live healthier. The Blueprint works to help Vermonters who have chronic conditions stay as healthy as possible – improving their quality and enjoyment of life and avoiding the need for complex care later when illness is harder to treat. The Healthier Living Workshops are sponsored by the Blueprint. These self-help workshops teach individuals to manage their chronic conditions and improve their health.  For information about the Healthier Living Workshops in our community, call Pam Smart at 802-748-7395.

For more information about the Vermont Blueprint for Health in the NVRH service area, contact Laural Ruggles at 802-748-7590.  Click here to read the latest Vermont Blueprint for Health Annual Report.
