Diabetes Self-Management Services

The NVRH Diabetes Self-Management Services provides support, tools, and education for accomplishing your personal health goals.

Diabetes Support Group – We meet every other month, on the last Monday of the month.  Guest speakers present various topics, as a way to stay current in your self-care.  A great opportunity to benefit from group support as you share experiences and information with other participants.  Guidance is also available in creating achievable action plans to fit your health needs.  Enjoy a light meal provided prior to meeting.

Where are you located?  The NVRH Diabetes Self-Management Services are located on the third floor of the hospital.  For information call Ginny or Erica at 802-748-7433. 

Who will I meet with?  In our programs you will meet with a Registered Dietitian and Certified Diabetes Educator and/or a Registered Nurse, who will work closely with your physician. 

Will my insurance cover visits or workshops?  Our services are covered by all insurances.  You will need a referral from your health care provider.  We recommend you check with your insurance to verify coverage and co-payments.  Our program is accredited by the American Association of Diabetes Educators.

What if I don’t have health insurance?  Please call 802-748-7526, Community Connections of NVRH.
